Cook County Vaccine sites are open for WALK-UPS and APPOINTMENTS.  Individuals or groups (ages 16+) who are interested in getting a vaccine, please see the details below

  • Monday – Saturday
  • Walk-Ups are welcomed at 7600 W. Roosevelt Road, Forest Park, IL 60130 (8:30AM – 5:00PM) 
  • Appointments are recommended to secure your dose.  Ages 12, 16 & 17 must be accompanied by a parent or guardian
  • Register online at vaccine.cookcountyil.gov      or
  • Call 833-308-1988  (existing cook county Covid-19 Vaccination registration number) Monday – Friday 7:00 AM – 10:00 PM

COVID-19 Testing

There are two kinds of COVID-19 tests.

  • Diagnostic test which detect virus and tell you if you have a current infection
  • Antibody test which might tell you if you had a past infection

There are two types of diagnostic tests: molecular tests, such as RT-PCR tests, that detect the virus’s genetic material, and antigen tests that detect specific proteins on the surface of the virus.
